A Unique Experience in Family Medicine
We call it “Total Care”, premium service and the highest level of care.
Planning Your First Visit
When scheduling a new patient appointment, keep in mind that we will require your arrival time to be 30 minutes prior to your appointment tom complete the new patient registration properly. If you are unable to arrive 30 minutes before your appointment, you may print a copy of our New Patient Registration (link to documents) packet, and fill it out ahead of time. Also, keep in mind the registration forms will need to be filled out completely before your appointment time whether you do it at home or in the office – whatever is most convenient for you.
Below you can click the button to access and print the New Patient Registration documents.
If you were recently in the Emergency Room, or Urgent Care, we will need the entire medical record (NOT discharge summary) prior to your visit to the clinic. Please come into the clinic or into the facility you were seen at, and sign a ‘Medical Release’ form to ensure we receive your records.
If you are coming in for a chronic condition follow up, the medical providers are requesting that your bring your current medication bottle, or a photo of the label.
Total Care begins when you drive up to the entrance of our state-of-the-art facility. Upon entering our clinic, you will immediately be welcomed by our competent, friendly medical staff.
Your medical concerns will be addressed quickly and effectively by our dedicated professionals.
Along with offering a full range of medical services, Total Care is also about maintaining family wellness through patient education.
At Total Family Care you will be treated with compassion and respect. We value our time and we value your time. We pledge to serve your family as we would serve our own.
